Inspector Notes
An unannounced, on-site renewal inspection was conducted on 9/24/2024, as a part of the licensure period. The on-site inspection began at 9:55am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 3 caregivers; the point total was 18. The inspector reviewed 3 children?s records, 2 staff records and 1 household member record on-site. This inspection included document review of policies and procedures, emergency drill logs, medication administration logs, emergency supplies, authorization forms,tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
5Caregiver #2 did not have documentation of trainings in the past 12 months.
Children's records are required to contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
The record for child #2 was missing parental authorization to administer prescription medications that were on site.
The record for child #1 was missing name, address and telephone number of two designated persons to contact in case of emergency and the parents cannot be reached.
There was not parent acknowledgement available for child #1 who has been in care for two months.
There was no documentation of immunizations for child #3, who has been in care for over a week.
