During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 3/24/2025, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 3 caregivers; the point total was 15. The inspector reviewed 4 children?s records, 3 caregiver records and 1 household member's record on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
7Household Member #1 did not have central registry results during the inspection and has been residing in the home for 45 days. Household member turned 14years old, over three months ago.
The provider's current CPR certifications was not from an approved agency.
The provider's current first aid certification was not from an approved agency.
The assistants' current CPR certifications were not from an approved agency.

Children's records are required to contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached
The record for Child #2 was missing the name, address and telephone number of a second designated persons to contact in case of an emergency, if the parent cannot be reached. The child was present during the inspection and had been in care for a month.
The phone number for emergency backup person was not posted.
