During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/30/2025, as a part of the licensure period. The on-site inspection began at 9:05am and ended at 10:35am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 caregivers; the point total was 34. The inspector reviewed 3 children?s records and 5 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION
Household members 14 years of age and older must obtain the results of a central registry
search within 30 days of turning 14 years old.

Household Member #2 did not have central registry results during the inspection and turned 14years old, over ten months ago. Central registry search was submitted 9/1/2025.
Plan of Correction: Central registry search was submitted 9/1/2025, awaiting results. Future background searches will be processed within a timely manner.

Before attendance, documentation of immunizations is to be obtained for each child, unless the parent submits an affidavit to the family day home on the current form approved by the Virginia Department of Health.

There was no documentation of immunizations or an affidavit exempting child from immunizations for child #2, who was in care during the inspection and had been in care for six months.
Plan of Correction: Provider will request required document from parent today. Provider will review files and documents for compliance.

Documentation of additional immunizations is required once every six months for children under the age of two years. '

Child #1, who turned two years old while in care, last documented immunization was over 12 months ago.
