Inspector Notes

An unannounced monitoring inspection was conducted from 10:15 - 11:30 am with the provider. There was one child in care, age 14 months, for a total of four points. The child was observed napping, waking, having a diaper change and having free play with a variety of age-appropriate toys. One child record, two caregiver records, written attendance, weekly menu and the first aid kit were reviewed. Required information was posted. The last emergency escape drill was conducted: 5/12/22 and the last shelter-in-place drill was conducted: 4/6/22. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Based on review of two caregiver records, it was determined that not all caregivers had an Office of Background Investigations (OBI) fingerprinting-based criminal history record check prior to their first day of employment. Based on observation, and an interview with the provider, it was determined that the home did not have a carbon monoxide detector as required. Evidence: the provider stated that the home does not have a carbon monoxide detector.

Based on review of documentation and an interview with the provider, not all records contained all required information. Evidence: the record for caregiver #2 (date of hire: 1/4/2022) did not contain the job title, date of hire, name, address and phone number of a person to be notified in an emergency, or documentation that two references as to character and reputation as well as competency were checked before employment.

Based on review of two caregiver records, the provider did not have documentation of a tuberculosis (TB) test/screening for a caregiver at the time of hire and prior to coming into contact with children.
