Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 6/3/24. There were 4 children present with 1 caregiver. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy
requirements that are not covered in licensing standard were reviewed during this supplemental inspection. Information gathered during the inspection determined violations with applicable standards or law. The violations are outlined on the violation notice.

Standard 8VAC20-790-170-G
The vendor shall maintain in a way that is accessible to all caregivers, a current written list of all children's allergies, sensitivities, and dietary restrictions, The list shall be dated and kept confidential.
There was no written list of child #1's allergy to shellfish and there was no written list of child #2's allergy to peanuts, tree nuts, diary and eggs.
Plan of Correction: I will write a list of those with allergies, sensitivities and dietary restrictions and post it with a coversheet. I will make sure the list is dated.
Standard 8VAC20-790-190-B-12
The vendor shall have a written care plan for each child with a diagnosed food allergy to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
There was no written allergy care plan for child #1 who has a diagnosed food allergy to shellfish. Child #1 has been enrolled for over 2 years.
There was no written allergy care plan for child #2 who has a diagnosed food allergy to peanuts, tree nuts, dairy and eggs. Child #2 has been enrolled for 5 months.
Plan of Correction: I will give the allergy care plan form to the parents to have the physician to fill out.
