Based on record review and staff interview the vendor did not ensure Staff who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training and staff development activities, to include the departments health and safety update course.
Evidence: Staff confirmed that Staff #1 (date of hire 7-12-2021) did not have documentation for their 2022 health and safety update course.
Plan of Correction: The director will ensure that staff #1 complete their health and safety course.
