Inspector Notes

A monitoring inspection was initiated and concluded on April 26, 2023. There were 40 children present, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on record review and staff interviews, the licensee did not ensure, all staff had obtained an out of state criminal history name check (CRC) prior to the first day of employment and an out of state child abuse and neglect search (CPS) was request by the 30th day of employment
Evidence: Staff #5?s record did not include an out of state CPS check or a CRC from California or a CRC from Louisiana where they had lived within the last five years from date of hire (DOH 3-7-22). Staff #5 was observed working during the inspection.
Plan of Correction: A request for the staff's out of state background checks were submitted . We're awaiting the results.
Standard 8VAC20-780-40-K
Based on review of center documentation and staff interviews, the licensee did not ensure the center shall develop written procedures for abusive head trauma.
Evidence: Center Director confirmed there was not a written abusive head trauma policy available for review.
Plan of Correction: The abusive head trauma policy has been written.
Standard 8VAC20-780-40-M
Based on observation and staff interview, the licensee did not ensure the center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
Evidence: Center Director, confirmed that there was not an allergy list available for staff to view for child #6 that has a physician documented peanut allergy.
Plan of Correction: Allergy list has been posted in each class.
Standard 8VAC20-780-430-J
Based on observation and staff interview, the licensee did not ensure that disposable products shall be used once and discarded.
Evidence: In the Preschool room the children were given disposable cups with their names on them to use for water during snack time. The center staff confirmed the disposable cups are used repeatedly during the day for water and are not thrown away after their first use.
Plan of Correction: The center's staff was trained on how to properly use disposable cups.
