Inspector Notes
A monitoring inspection was initiated and concluded on April 26, 2023. There were 40 children present, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
4Evidence: Staff #5?s record did not include an out of state CPS check or a CRC from California or a CRC from Louisiana where they had lived within the last five years from date of hire (DOH 3-7-22). Staff #5 was observed working during the inspection.
Evidence: Center Director confirmed there was not a written abusive head trauma policy available for review.
Evidence: Center Director, confirmed that there was not an allergy list available for staff to view for child #6 that has a physician documented peanut allergy.
Evidence: In the Preschool room the children were given disposable cups with their names on them to use for water during snack time. The center staff confirmed the disposable cups are used repeatedly during the day for water and are not thrown away after their first use.
