Complaint Related
Any person providing full-time or part-time child care for pay on a regularly planned basis have reason to suspect that a child is an abused or neglected child, shall report the matter immediately to the local department of the county or city wherein the child resides or wherein the abuse or neglect is believed to have occurred or to the Department's toll-free child abuse and neglect hotline. The center did not report suspected child abuse from Staff #1. Staff f#2 indicated Staff #1 was no longer employed because they had determined they had engaged in forbidden action and their action was suspected it to be child abuse.

Plan of Correction: Staff #2 informed the parent immediately of the incident and the parent of child #1 reported. However,Staff #2 will properly report any future abuse.

Repeat Violation
The required ratio for children to birth to 16 months is one staff per four children. The following was observed, and the ratio was not met. On 10-29-2025, Staff #3 was alone supervising 5 children in Classroom A to include Child #2, age 10 months. There was only one staff supervising children for approximately 1 hour.

Plan of Correction: Staff #3 as well as staff that left #3 out of ratio received training on the importance of ratios as well as a disciplinary action.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. The center did not comply with their own policies listed in their parent handbook.

Complaint Related
Staff #1 was observed striking and roughly handling Child #1. As Child #1 was crawling away, Staff #1 picked up a plastic container and hit the child on the head. Child #1 immediately turned and grabbed the back of his head in response. About three minutes later, Child #1 was seen pushing another child. Staff #1 approached, kicked her leg toward Child #1 and Child #1 fell. Staff #1 then swatted at the child with an open hand. When attempting to pick up the child, he kept falling to the ground. Staff #1 then grabbed his arm while he was lying on the floor and dragged him out of the classroom into the hallway.
