When staff are supervising children, they shall always ensure their care, protection, and guidance. Child #2 was observed hitting and pushing children in the classroom, and throwing toys/items at staff and other children. Child #2 used both hands and punched Child #3 in the chest and then Child #3 immediately vomited. Staff did not intervene for approximately 8 minutes.

The required ratio for children age two is one staff per eight children. Staff #1 was observed alone supervising 10 children in Classroom C to include Child #1, age 2. The ratio was not met in Classroom C for approximately 1 ? hours.
