Based on document review and discussion, the vendor failed to ensure that the written hard copy of daily attendance documents the arrival and departure of each child in care as it occurs.

Evidence: The written hard copy attendance record did not document the arrival and departure of each child as it occurred.
Plan of Correction: Will revise the form and will document arrival and departure as it occurs.
Standard 22VAC40-665-520-B-11

Evidence: The record for Child 2 contained documentation of a diagnosed food allergy. There was not a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction in the record for Child 2.
