Inspector Notes
An unannounced monitoring inspection was conducted on 01/30/2024 in response to a self-reported incident. There was a total of 32 children and 7 staff members present during the inspection. The inspector reviewed photographs, interviewed two staff members and reviewed a total of 1 child records and 1 staff record during the investigation. The inspector arrived at 11:45 am and departed at 12:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
2Evidence: On 01/25/2024 Staff 2 was notified that upon arriving at home from care that day, the parent of Child 1 (age 9 months) discovered that Child 1?s Velcro bib had been tied tightly in a double knot around their neck. Pictures of Child 1 show that the bib was double-knotted and tied tight enough to leave a visible red mark around the child?s neck. It was determined through staff interviews that Staff 1 had tied the bib around Child 1?s neck. Staff 1 did not ensure the care and protection of Child 1 when she tied the bib tightly around the child?s neck.
Evidence: There were 16 children and 2 staff members observed in the Toddler-2?s Classroom during the 01/30/2024 inspection. Child 2 was determined to be 17 months old. The required staff-to-child ratio for children 16 months- 24 months is 1:5. Three staff would be required for this group of children.
