An unannounced on-site focused inspection was initiated on 09/30/2025 and completed on 10/02/2025. The inspection included document review, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center shall inform the department?s representative as soon as possible, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Child 1 (age 3 years) received an injury that required outside medical attention on 09/17/2025; the center did not inform the department?s representative within two business days of the injury that required outside medical attention.
