Inspector Notes

A complaint inspection was initiated on May 11, 2022 and concluded on May 23, 2022. A complaint was received in the Office of Child Care Health and Safety (Valley) on April 13, 2022 in regards to administration, physical plant, programs and nutrition. An on-site inspection was completed on May 19, 2022. There were 20 children present, ranging in ages from two to five years of age, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming and nutrition. One child record was reviewed and two staff were interviewed.

There was only preponderance of evidence for part of the complaint, therefore the complaint is considered valid. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
Based on review of child records, the center failed to ensure immunizations were obtained prior to the first day of attendance.

Evidence:
1. Child #1's date of enrollment was 02/28/2022. The immunizations on file were dated 03/11/2022.
2. Administration verified the date of enrollment and the date the immunizations were obtained.
Plan of Correction: Administration had requested the documentation prior to the child attending, and had to ask several times to obtain the documentation. The documentation was not handed to us, but put in an outside drop box. In the future a child will not be able to attend until all required paperwork is on file.

Based on review of children records, the center failed to obtain all required information.

Evidence:
1. One of the emergency contacts listed did not have a phone number documented.
2. Both of the emergency contacts listed did not contain information regarding their address.
3. Administration verified that the emergency contacts did not have the required information in the child's file.
Plan of Correction: The child is longer enrolled in the center. In the future a child will not be able to attend until all required paperwork is on file and has been reviewed by administration for completeness..
