Inspector Notes
An unannounced subsidy health and safety inspection (SHSI) was initiated and concluded on Tuesday, September 13, 2022. The licensing inspector was on site from approximately 10:10am until 11:40am. There were two children present with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and programming. A total of four children?s records, two caregiver records, and one household member?s records were reviewed. A monitoring inspection was also conducted on this date. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
6Evidence: 1) The record for Caregiver #2, employed 08/02/22, did not contain the results of a fingerprint-based national criminal background check. 2) During interview, Caregiver #1 reported the fingerprints for Caregiver #2 have not been requested.
Evidence: 1) The record for Caregiver #2, employed 08/02/22, did not contain the results of a Virginia Child Protective Services Central Registry check. 2) During interview, Caregiver #1 reported the Virginia Child Protective Services Central Registry check has not been requested.
Evidence: 1) The record for Caregiver #2, date of employment 08/02/22, did not contain a completed sworn statement. 2) During interview, Caregiver #1 reported Caregiver #2 has not completed a sworn statement.
Evidence: 1) The record for Caregiver #2, employed 08/02/22, did not contain a TB test or screening. 2) During interview, Caregiver #1 reported the TB for Caregiver #2 has not been received. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department .
Evidence: 1) The record for Child #1, date of attendance 09/12/22, did not contain documentation of immunizations. 2) The record for Child #2, date of attendance 05/09/20, did not contain documentation of immunizations. 3) During interview, the provider reported the immunizations for Child #1 and Child #2 have not been obtained.
Evidence: 1) The record for Caregiver #2, employed 08/02/22, did not contain documentation of orientation training. 2) During interview, Caregiver #1 reported Caregiver #2 has not completed orientation training.
