An unannounced subsidy health and safety inspection (SHSI) was initiated and concluded on Tuesday, September 13, 2022. The licensing inspector was on site from approximately 10:10am until 11:40am. There were two children present with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and programming. A total of four children?s records, two caregiver records, and one household member?s records were reviewed. A monitoring inspection was also conducted on this date. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of two caregivers' records and interview, the vendor did not ensure one caregiver obtained a fingerprint-based national criminal background check prior to employment.
Evidence: 1) The record for Caregiver #2, employed 08/02/22, did not contain the results of a fingerprint-based national criminal background check. 2) During interview, Caregiver #1 reported the fingerprints for Caregiver #2 have not been requested.

Based on a review of two caregivers' records and interview, the vendor did not obtain the results of a Virginia Child Protective Services Central Registry check for one caregiver by the end of 30th day of employment.

Based on a review of two caregivers' records and interview, the vendor did not ensure one caregiver was evaluated by a health professional and be issued a statement that individual is determined to be free of communicable tuberculosis (TB).
Evidence: 1) The record for Caregiver #2, employed 08/02/22, did not contain a TB test or screening. 2) During interview, Caregiver #1 reported the TB for Caregiver #2 has not been received. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department .

Based on a review of four children's records and interview, the vendor did not obtain documentation that two children obtained immunizations according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations before the child attended the family day home.
Evidence: 1) The record for Child #1, date of attendance 09/12/22, did not contain documentation of immunizations. 2) The record for Child #2, date of attendance 05/09/20, did not contain documentation of immunizations. 3) During interview, the provider reported the immunizations for Child #1 and Child #2 have not been obtained.

Based on a review of two caregivers' records and interview, the vendor did not complete orientation training for one caregiver prior to the caregiver working alone with children and within seven days of the date of employment.
