During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was completed on 07/10/2025, as a part of the licensure period. The on-site inspection began at 8:00 am and ended at 9:40 am. The inspector reviewed compliance in the areas listed above. There were 18 children present and 5 staff. The inspector reviewed 4 children?s records and 2 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

The center did not have an updated sensitivity listed on the allergy list and the list was not covered to keep information confidential. A child's identifying information was pulled above the coversheet hanging on the wall.
Plan of Correction: We will check with the child's mom. He does not have anything listed with us .
Standard 8VAC20-780-60-A
The records for children shall contain documentation of child updates.

Child A's annual record update was approximately 2 months over due.
Child B's annual record review was approximately 1 month overdue.
Plan of Correction: We will get updates and create a sheet for students that ride the bus next year.
