Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on Friday, May 21, 2021 and concluded on Tuesday, May 25, 2021. The director was contacted by telephone to initiate the inspection. There were 24 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard 22VAC40-185-160-A
Based on review of staff record and interview, the center did not ensure to obtain documentation of a tuberculosis screening within 12 months prior to or 21 days after employing or volunteering.

Evidence: In Staff #2's record (date of hire: 08/09/18) there was no documentation of TB screening found. Interview with staff confirmed the TB screening documentation was not available for review during this inspection.
Plan of Correction: To make sure all staff have their, TB papers in on the first day of work.
Standard 22VAC40-191-60-C-2
Based on review of staff record and interview, the center did not ensure to obtain a central registry finding within 30 days of employment or volunteer service.

Evidence: In Staff#1's record (date of hire: 03/02/2021) there was no central registry finding found. Interview with staff confirmed the central registry finding was not available for review during this inspection.
Plan of Correction: Make sure all new hire paperwork is in before they start to work.
