Inspector Notes
A monitoring inspection was initiated on August 2, 2022 and concluded on August 5, 2022.
There 46 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
4Evidence: In Staff#1's record (date of hire: 05/31/22), there was no out of state central registry results found for review. Interview with staff confirmed the request for central registry search to obtain results had not been submitted at this time.
Evidence: In Staff#2's record (date of hire: 06/30/20), there was no documentation found of a negative TB screening for review. Interview with staff confirmed the documentation not signed by a physician was available for review during the inspection.
Evidence:
1. In the Fours/Fives classroom, the thermostat was observed off and one small fan and medium fan were observed on, however, the room was deemed very hot inside.
2. Interview with staff in classroom and administrators, confirmed the AC unit was not working properly since Friday and the thermostat was turned of as a result. Interview with staff revealed AC issues existed prior to Friday and it has been ongoing since the start of summer.
3. On 08/02/22 a follow up inspection conducted by inspector, the temperature in the Fours/Fives classroom read 84 degrees. The children remained out of the classroom until completion of maintenance repairs.
Evidence:
1.In the School Age classroom, there was a cabinet unlocked in the bathroom area containing disinfectant wipes with the labels, "caution" and "keep out of reach of children".
2. In the Fours/Fives classroom, there was fragrance spray with the labels "caution" and "keep out of reach of children".
3. In the kitchen area, there was multiple disinfectant wipes observed on the counter top with the labels "caution" and "keep out of reach of children".
