Inspector Notes

A monitoring inspection was initiated on January 6, 2023 and concluded on January 10, 2023.
There were 27 children present, ranging in ages from infancy to 4 years, with 6 staff supervising. The
inspector reviewed compliance in the areas of administration, physical plant, staffing and
supervision, programming, special care and emergencies and nutrition. A total of
3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-350-B-1
Based on observation and interview, the center did not ensure to follow the staff to children ratios for birth up to 16 months as required.

Evidence: During the inspection, in the Butterflies (Infant) room, there was observation of one staff present with five infants in care. Interview with staff confirmed the ratio for this group is 1:5.
Plan of Correction: I moved a Child to the toddler room. Ratio is 1:4 now.

Based on observation, center did not ensure the diapering surface use for only diapering.

Evidence:
1. During the inspection, in the Butterflies room the diapering surface was observed with a face mask stored on top with visible stains on the surface.
2. Photographs of the diapering surface observation during the inspection.
Plan of Correction: Have spoken to the staff person to let know nothing is to be on changing table and to make sure to keep it clean always.
