Standard 22.1-289.011-F
(Repeat Violation) The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises. The most recent inspection findings were not posted at the time of the inspection.
Plan of Correction: The program Director will post the most recent inspection findings now and will update the posting with new inspection findings after receiving each inspection report form the licensing inspector following each visit.
Standard 8VAC20-780-160-A
Each staff member must submit documentation of a negative tuberculosis screening.at employment and before coming into contact with child. The documentation have been completed within the last 30 calendar days of the date of employment.
The documentation of Tuberculosis screening for staff #1 was dated more than 8 months prior to their date of employment with the center.
Plan of Correction: The program director will ensure that TB screening documentation submitted by staff at the time of hire has been completed within the last 30 calendar days of the date of employment.
Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff must obtain and submit the results of a follow-up tuberculosis screening.
Staff #4 had documentation of the last tuberculosis screening which was more than 15 months overdue.
Plan of Correction: Staff #2 will be required to obtain an updated TB screening and the results will be submitted to the program director by no later than the end of the month.
Standard 8VAC20-780-240-B
(Repeat Violation) Staff shall complete orientation training prior to working alone with children and no later than seven days of the date of assuming job responsibilities.
Four of five staff records reviewed did not contain documentation of staff completing orientation within 7 days of assuming their job responsibilities.
Staff #1, who works caring for children in a classroom, obtained orientation14 days late.
Staff #2, who works caring for children in a classroom, obtained orientation 7 days late.
Staff #3, who works caring for children in a classroom, did not have documentation of having completed orientation training which was overdue by more than 4 years.
Staff #5, who works caring for children in a classroom, did not have documentation of having completed orientation training which was overdue by more than 3 years.
Plan of Correction: The orientation for staff #3 and staff #5 will be reviewed and new documentation of orientation training will be placed in their records. Orientation was conducted when they were hired but the documentation could not be located.
Orientation for new staff will be conducted within 7 days of hire and prior to staff working alone with children and documentation to reflect the time of hire and orientation will be placed in each staff's record for review.
Standard 8VAC20-780-560-F
(Repeat Violation) When centers choose to provide meals or snacks centers shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). A menu listing foods to be served for meals and snacks during the current one-week period must be posted and list any substituted food.
Three classrooms of children were observed to have been served a snack which did not meet requirements of the Child and Adult Care Food Program of the United States Department of Agriculture. In addition, the snack served did not match what was listed on the menu posted at the front of the facility for parents to reference.
The Toddlers and Two year old classroom served apple juice, cupcakes and cookies for snack.
Preschool children were served cookies and a beverage whose first 2 ingredients were filtered water and sugar.
The school aged classroom was served graham crackers and apple juice.
The posted menu listed wheat thins and cheese sticks to be served for snack and remained unchanged at the time of the inspection concluding at 4:30 pm.
Plan of Correction: The program Director went to the kitchen and removed cheese sticks from the freezer, defrosted them in the microwave and brought them to the classrooms for staff to provide the children.
The program director will advise classroom staff that they should serve the food listed on the menu or consult admin for appropriate substitutions if that snack is not available. In the event that a substitute snack must be served, the menu(s) available for parents to review will be updated to list the substituted food(s) at the time that snack is served.
