During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/24/2026. The on-site inspection began at 9:15 am and ended at 11:40 am. The inspector reviewed compliance in the areas listed above. There were 23 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/2/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance. There was no physical examination in the record of child #3, who had been enrolled for three months.
Plan of Correction: Student is no longer part of center. Last day was 3/25/2026.
Standard 8VAC20-780-380-A
There shall be a posted daily schedule. There was no posted daily schedule in the Pre-school room.

Standard 8VAC20-780-500-B
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. Staff #3 cleans the surface after each use and sanitizes it once at the end of the day.
Plan of Correction: New changing pads were ordered for both class rooms.
Standard 8VAC20-780-570-E
Prepared infant formula shall be refrigerated, dated and labeled with the child's name. There were five bottles in the refrigerator in the infant room. None of them were dated or labeled with the child's name.

Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for twelve months, was missing parents employment information. The record for Child #2, enrolled for five months, was missing one parents employment information.
