Inspector Notes

A religious exempt renewal inspection was conducted on April 25, 2022. There were 30 children present, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision. A total of 10 child records and 8 staff records were reviewed. Based on review of 8 staff records, 1 record did not contain documentation of submitting to fingerprinting.
Evidence
Staff 31 did not submit to fingerprinting before first day of employment.
Plan of Correction: Staff #1 will have their fingerprinting done and the eligibility letter will be placed in their record. I will send the letter to the inspector when it is received. Going forward, all staff will have their fingerprinting done before their first day of employment.

Based on review of 8 staff records, 3 did not submit to central registry background checks.
Evidence
The record for staff #1, #2 and #3 did not have central registry background check results within 30 days of their date of hire.
Plan of Correction: Staff will submit central registry background checks. The results will be placed in their record. I will place the results in their record when they are received. Going forward new staff will submit their central registry checks their first week.
