Inspector Notes

A /renewal inspection was conducted today between 9:00am and 10:30am. There were 43 children present, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 7 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observation, the child day center did not post the fact that it is exempt from licensure in a visible location on the premises.
Evidence
The child day center did not have the notice of exempt from licensure posted.
Plan of Correction: I didn't know it need to be posted. Notice will be posted today.
Standard 22.1-289.035-B-2
Based on review of staff records, there is not documentation of submission to fingerprinting for all staff.
Evidence
Staff #1, first day of employment 4/23/2023, does not have documentation of submission to fingerprinting. There is no eligibility letter in their record.
Plan of Correction: Staff will make an appointment and have the fingerprint completed. I will send the inspector the results when received.
Standard 8VAC20-770-40-D-2
Based on review of staff records, the center does not have documentary evidence that the center is in compliance of submitting a search of central registry within 30 days of the first day of employment.
Evidence
Child #2, first day of employment 8/24/2018, does not have documentation of a central registry search.
Plan of Correction: The CPS form was sent to the staff member instead of the center, she has already completed the new form. I will submit the form today. I will send the results to the inspector when received.
