Inspector Notes
An unannounced renewal inspection was conducted today from 11:20am-12:25pm. There were 9 children directly supervised by 3 staff. The home, 3 staff records, 5 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing freely and eating snack. There was an adequate supply of books, toys, and materials for the children. The home was clean and organized. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

3Evidence:
Two unused outlets were observed without a child-resistant protective covers in the napping room, one next to the light table and one next to a crib.

Child #3's file has an authorization for emergency medical care that was not signed by the parent.
Evidence:
The plastic covering for the diaper changing mat had a tear, approximately 2 inches long, and therefore is no longer nonabsorbent
