Each report of immunizations shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. The record of child #1 contained documentation of immunizations that were not dated or signed by a physician. 2. The center shall ensure when any child for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed, shall always be in the care of a staff member or independent contractor who meets the requirements. Child #1 prescribed an emergency medication was in care on February 18, 2026. Staff #1 and staff #3, the only staff on site, did not have MAT certification.

The training shall include an in-person competency demonstration. Staff #1 and staff #3 were observed with a group of children. Neither staff #1 or staff #3 had current CPR certification.

However, staff who are registered nurses or licensed practical nurse with a current license from the Board of Nursing shall not be required to obtain first aid certification. 1.
