Inspector Notes

An unannounced monitoring inspection was conducted to investigate a self-reported incident related to the forbidden action of roughly handling a child at the center. The investigation was conducted in conjunction with investigation from the local agency and law enforcement. The inspection was initiated with the director on July 27, 2023 beginning at 1:00 PM and ending at 1:53 PM. An exit meeting was conducted on August 8, 2023. One staff record, one child's record, documentation, and video recordings were reviewed. Staff interviews were completed. Additional information was gathered from the local agency and law enforcement. A violation was issued as a result of this monitoring inspection.

Standard 8VAC20-780-410-1
Based on a review of video recording and staff interviews, a staff member utilized the forbidden action of roughly handling a child at the center. Evidence: Staff 1 was observed on the video recording standing at the open doorway of the classroom when child A crawls through the open doorway. Staff 1 is observed picking child A, age 12 months, up by one arm and place him on the floor inside the classroom twice. The second time staff 1 picked child a up by the arm it was with the enough force that his legs swung from side to side before he was placed on the floor.
Plan of Correction: The staff member was terminated immediately as a result of the incident. The staff members received re-training on how to appropriately pick up a child using two hand and under the arms. The staff also received additional training on behavioral management.
