During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/25/25. The on-site inspection began at 9:05am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There were 17 children present and four staff. The inspector reviewed four children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/6/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-140-A
The center is required to obtain a physical examination by or under the direction of a physician for each child within 30 days after the first day of attendance.

The record for Child A, who had been enrolled for more than three months, did not contain a physical.
Plan of Correction: Child A is a foster child and we will sure to comply with the regulations. We already asked for a copy of the physical and it's filed.
Standard 8VAC20-780-80-A
The center is required to maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs.

On 9/25/25 at approximately 9:25am there were six children observed in the classroom for two- and three-year-old children. The attendance record contained documentation of only two children present.
Plan of Correction: We are training our new staff and making sure that they understand the importance of maintaining the children's attendance record correctly.
