Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 09/15/2023, between the hours of 9:30 am to 10:30 am. There were 3 children present (8 points), ranging from infant to preschool-age, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 5 children records, 1 staff record, and 3 household records were reviewed.

Information gathered during the inspection determined that there was (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard 8VAC20-800-180-A
Based on review, 1 of 3 household member files indicated that the Provider did not obtain for each adult household member a current Report of Tuberculosis Screening form(in accordance with the requirements in 8VAC20-800-170), every two years.

Evidence -
1. On the date of inspection (09/15/2023), TB documentation for Household Member #2 was dated - 08/19/2021.
2. This was confirmed on the date of inspection by the Provider.
Plan of Correction: Updated TB documentation will be obtained for Household Member #2 and placed on file.
Standard 8VAC20-800-460-C
Based on review and interview, 1 of 4 household pets did not have a current rabies vaccination.

Evidence -
1. On the date of inspection (09/15/2023), Pet #1 had documentation that indicated that its rabies vaccination expired on 08/18/2023.
2. The Provider stated (during an interview conducted on the date of inspection) that the animal was ill during the time set for the renewal of the vaccination, so the Provider had to wait.
Plan of Correction: Pet #1 is scheduled to have this vaccination administered next month.
