Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 09/27/2024 and completed on 09/27/2024. The on-site inspection began at 9:30 am and ended at 11:30 am. The inspector reviewed compliance in the areas listed above. There were (4) children present and (2) caregivers; the point total was (11). The inspector reviewed (5) children?s records and (2) caregivers, (3) adult household member records on-site on 09/27/2024.

This inspection included: document review (policies and procedures, emergency drill logs); tour of the facility; and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.036-B-2
An authorize family day home (FDH), obtain a copy of the results for fingerprinting (FPC) and provided personal descriptive information for all caregivers and adult household members every five years.

Caregiver #2 had documentation of a FPC on-site that had expired 4 months ago.
Plan of Correction: A FPC will be obtained and placed on file. The LI will be alerted to when the check has been requested and received.

Authorize family day home (FDH) obtain a copy of the results of a search of the central registry (CPS check) for any founded complaint of child abuse or neglect against any adult household member or caregiver every 5 years.

Caregiver #2 had documentation of a CPS check on-site that had expired 3 months ago.
Plan of Correction: A CPS check will be obtained and placed on file. The LI will be alerted to when the check has been requested and received.

Caregivers shall obtain a minimum of 16 clock hours of training annually.

Caregiver #1 obtained 8.5 training hours in 2023.
