During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 03/13/2026. The on-site inspection began at 10 am and ended at 11 am. The inspector reviewed compliance in the areas listed above. There were (5) children present and (2) caregivers; the point total was (12). The inspector reviewed (6) children?s records, (2) caregivers records, and (3) household member records on-site on 03/13/2026.

This inspection included: document review (i.e. Files, injury logs (none), policies and procedures, and emergency drill logs), tour of the facility, interviews, and observations.

Information gathered during the inspection determined (1) non-compliance with applicable standard. This violation was documented on the violation notice issued to the home.

Documentation of additional immunizations is required once every six months for children under the age of two years. '

Child #5, 22 months old, last documented immunization was over 7 months ago.
Plan of Correction: Will contact parent and obtain updated documentation after the child's next scheduled check-up.
