Inspector Notes

An unannounced monitoring inspection was conducted on 8/11/2022. There were no children registered at this time, this center is used for schools out programs. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. 2 staff records were reviewed. The inspector arrived for the inspection at 9:00am and departed the center at 9:45am.

Discussion on shaken baby or abusive head trauma procedures.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on the review of two staff?s records, the center failed to ensure that a follow up tuberculosis screening for staff members was obtained.

Evidence: Staff #1 was hired on 12/15/2014 and on the day of the inspection the most recent tuberculosis screening expired 8/29/2019.
Plan of Correction: Provider will obtain a copy of the tuberculosis screening and place in staff record.
