Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 12/5/2024 and completed on 12/5/2024. There were 11 children and three staff present. The inspector reviewed three children?s records and four staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Program leaders shall have fulfilled a high school program completion or the equivalent and have meet one of the following experience and education requirements. The file for Staff 2 shows they are a lead, however there is no documentation of education or experience to show how they are qualified. The file for Staff 3 shows they are a lead, however there is no documentation of education or experience to show how they are qualified.
Plan of Correction: The center will either change the job title, or obtain the documentation and put it in the file.
