The center is required to obtain a completed sworn statement prior to the employee's first day of employment. The sworn statement for S3, who is currently working, was dated 23 days after their first day of employment.
Plan of Correction: The center has updated it's orientation process to obtain a completed sworn disclosure statement prior to the employee's first day of employment. Pre-Service training (which is paid time) will not start until all background checks are received.
