Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A renewal inspection was initiated and concluded on 11/22/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 2 children present (5 points), ranging in ages from 23months to 2years, with the provider supervising. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 children's records and the provider's record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and one violation was documented on the violation notice issued to the program.

Based on a review of caregiver health records, it was determined the provider did not obtain a current Report of Tuberculosis Screening form, in accordance with the requirements in 22 VAC 40-111-170, every two years from the date of her first screening.
Evidence:
The provider's TB Health Screening expired on 5/3/2021.
Plan of Correction: I will make an appointment for a new TB screening.
