Inspector Notes
An unannounced renewal inspection was conducted at 10:08am. Observed 5 children totaling 11 points with 1 caregiver. Ratios and supervision were in compliance. Children were observed eating snack, washing hands, playing with toys, drawing, listening to a holiday story , having creative play with a cardboard box and singing to music and dancing. Snack served included blueberries, vegetable snack sticks, milk and water. The caregiver was actively engaged with the children and activities. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Violations
7Based on records reviewed, there was no documentation that 1 of 2 caregivers completed a TB screening every 2 years. Evidence: The previous TB test for Caregiver #1 was dated 11/24/21.
Based on records reviewed, 3 of 5 children's records did not include required information.
Evidence:
1. Child #3- The termination policy was not written into the document signed by parents. It was left blank.
2. Child #4- The termination policy, daily schedule, arrival/departure procedures and the emergency plan were not written into the document signed by parents. Those areas of the were left blank.
3. Child #5-The termination policy , daily schedule, behavior management time-out policy and the emergency plan were not written into the document signed by parents. Those areas of the were left blank.
Based on records reviewed, 2 of 5 children's records did not include the parents signed acknowledgement of reviewing emergency contact information annually.
Based on records reviewed, 2 of 5 children's records did not include documentation of immunizations every 6 months until the age of 2 years old.
Evidence:
1. Child #1-The immunizations on-file were dated September 2022 when the child was 4 months old.
2. Child #2-The immunizations on-file were dated July 2021 when the child was 5 months old.
