During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site renewal inspection was conducted on 12/6/24. The on-site inspection began at 9:45am and ended at 11:20am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver; the point total was 9. The inspector reviewed 5 children?s records and 3 caregiver/household members records while on-site. The inspection included document review, tour of the daycare areas of the home and interview.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/18/24. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION
The violation notice from the most recent inspection shall be posted in a conspicuous place on the licensed premises.

The violation notice from the 6/12/24 inspection report was not posted.
Plan of Correction: Correct my mistake, print the correct paper and post it up.

REPEAT VIOLATION
Licensees and household members shall obtain background checks every 5 years.

The Central Registry Searches for the licensee, and household members #1 and #2 were last completed 6/5/19.

Documentation of a physical exam shall be obtained for each child within 30-days after the first day of attendance.

The record for Child #7 did not include a physical exam.

Before the child's first day of attendance, parents should be provided in writing the policies and procedures, to include the termination policy.

The records for children #7, #8, #9, and #10 did not include documentation of the termination policy.
Plan of Correction: Insure to provid [sic] termination policy to children #7, #8, #9, #10 and make sure to continue to include the termination policy in the future.

A working battery-operated portable weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times.

There was not a weather band radio on-site. The provider did not know where it was located. .
Plan of Correction: Buy new better weather band radio and keep in a easy access/rememberable place w/ extra batteries

Documentation of immunizations shall be obtained once every 6 months for children under the age of 2 years old.

The immunizations for Child #7 were dated one year ago.
