When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 11/11/2025, Child #1, (four years old), ran out of the classroom. Child #1 ran through two doors located inside of the building, and out of a door that led to outside. Video footage dated 11/11/2025 showed Child #1 run out of the entrance door and crossed the parking lot into the business parking lot that is next to the center. The main road in front of the center is approximately 88 feet from the entrance of the building, and the speed limit is 45 mph. Staff #3 retrieved the child and brought her back to the facility.
Plan of Correction: On 11/11/2025, Child #1 was asked to place an AirTag back into her cubby after
she was observed playing with it during class. As the child began walking toward the carpet area she abruptly ran toward the classroom door. The teacher positioned closest to the door attempted to stop her but was unable to reach her before she exited the classroom.

The child proceeded to run through two interior doors leading toward the front of he building. Upon reaching the entrance area, she was immediately observed by the center director, who pursued her as she moved toward the exterior exit. Staff acted promptly and the child was never out of sight/sound supervision; however due to the child's speed and the element of surprise, they were unable to prevent her from exiting the main entrance. Staff #2 retrieved lite child from the parking lot and safely returned her to the building.

-he family was notified of the incident the same day. Due to safety concerns the child was dismissed from the program to ensure her well-being and the
safety of the group.

To strengthen supervision practices and prevent future incidents, the following steps have been put in place:

Classroom Door Positioning
When two staff members are assigned to a classroom, one teacher will maintain a position near the classroom door whenever possible to minimize the likelihood of a child exiting without permission.

Increased Front-Entry Monitoring
Directors and management will maintain heightened presence in the front-entry area during peak transition periods to support immediate response needed.

"Runner Alert" Procedure
A new internal procedure has been implemented requiring staff to loudly announce, "We have a runner!" so nearby staff members can immediately assist in blocking exits and supporting supervision.

Staff Retraining
All staff will undergo retraining on:
-Active supervision strategies
-Classroom and doorway positioning
-Emergency response protocols
-Quick-response communication procedures

These steps are being taken to strengthen supervision practices, reinforce staff awareness, and ensure the continued protection of all children in our care.
