Inspector Notes

This inspection was conducted by staff using an alternative remote protocol, including phone interview, records review, and a virtual observations. An on-site inspection was not conducted at this time.

An unannounced monitoring inspection was conducted at 10:11am. The provider was contacted by phone to initiate the inspection. There were 5 children in care + 1 child that lives in the home = 18 points with the provider and assistant #1. Ratios and supervision were in compliance at the time of inspection. Children were observed eating a morning snack. The home was well supplied with toys and equipment for the children, and appeared clean and organized today. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Based on interview conducted and records reviewed, it was determined that the Provider did not have a required out-of-state background check. Evidence: The Provider's Sworn Disclosure Statement (SDS), dated 5/9/19, indicated that she had lived out-of-the State of Virginia in the previous 5 years however, a sex offender registry search for that State had not been completed to-date.
Plan of Correction: On 3/8/22, the Provider completed the sex offender registry search, as well as, an updated SDS.
