Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 8/16/24. The on-site inspection began at X10:40am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 5 children present, plus 1 child that lives in the home and 2 caregivers. The inspector reviewed 5 children?s records while on-site. This inspection included document review, tour of the daycare areas of the home and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat violation.
Based on records reviewed, 3 of 5 children's records did not include required information.

Evidence:

1. Child #10- There was no allergy care plan for the documented food allergies, or updated health information indicating the food allergies no longer exist.
2. Child #11- There was no allergy care plan for the documented food allergies, or updated health information indicating the food allergies no longer exist.
3. Child #13-The parent did not sign the emergency medical authorization.
Plan of Correction: Caregiver #1 said the enrollment paperwork will be completed.
Standard 8VAC20-800-90-A
Based on observations made, 1 of 5 children's records did not include documentation of immunizations prior to the child's attendance. Evidence: Child #13-There were no immunizations on-file. The child began attending 8/5/24.
Plan of Correction: Caregiver #1 said the parents will submit the needed information.
Standard 8VAC20-800-900-B
Based on observations made, children were observed walking while eating today. Evidence: 4 children were observed walking out of the house for outdoor play while eating bagels.
Plan of Correction: After the inspector brought this to the caregivers attention, the children were seated at a table outside to finish eating before playing.
