Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 9/11/2024. The on-site inspection began at 10:25am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 12. The inspector reviewed 2 children?s records and 1 caregiver and 1 household member records on-site.
This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/8/2024. A POC submitted after this date will not appear on the public website.

8Staff #1's updated central registry search results and fingerprint-based background checks were due over three months ago. Staff #1's updated sworn disclosure statement was due over one month ago.
Staff #2's updated central registry search results were due over three months ago.
Staff #1's most recent TB screening was completed two years and seven months ago. Household Member #1's most recent TB screening was completed two years and two months ago.
The provider has not been taking daily attendance for at least eight months.
Records for Child #1 and #2 did not have a parent's signature for an annual review of the emergency contact information. Child #1 and #2 and have been in care for over one year and eight months.
Emergency evacuation drills have not been conducted monthly.
There was no documentation of any emergency evacuation drills being conducted.
Child #2, 22 months old, last documented immunizations was over 13 months ago.
