This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and an interview with the director.
An unannounced monitoring inspection was initiated on January 14, 2022 to investigate a self reported incident regarding a forbidden action. Based on evidence obtained through interviews and a review of documentation, the self reported incident could not be substantiated. There were no violations issued as a result of the inspection.
