During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 8/12/2025 in response to a self-report received by the licensing office on 7/30/2025 relating to forbidden actions. The inspector reviewed five children?s records and five staff records on-site. This inspection included: document review (policies and procedures, medication authorizations, background checks, training documentation, and complaint documentation), tour of the facility, interviews, and observations of a video recording.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/27/25. A POC submitted after this date will not appear on the public website.
Violations
3The Center?s ?Employee Handbook? states that ?roughly handling a child? is a ?Prohibited Discipline/Action?. Staff #1 was observed on a video recording dated 7/29/25, picking up Child #1 (age 3) by both arms above the elbows. Staff #1 was face to face with Child #1 as she attempted to move/drag the child to a table. Staff #1 walked forward with Child #1 who was backwards while holding her above both elbows. Child #1 went limp so Staff #1 lifted and dragged her roughly before letting go of the child. Staff #3 and Staff #4 reported that the child had blotchy red marks above her elbows after the incident.
