During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Inspector Notes
An unannounced monitoring inspection was initiated and completed on 12/16/25, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the inspection dated 8/12/25 related to Administration and Programs. The inspector reviewed one child?s record and three staff records on-site. This inspection included document review (i.e. staff training, and policies and procedures), tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
