Inspector Notes
A monitoring inspection was conducted on 11/20/2023. There were 7 children present, totaling 14 points, with the provide supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 provider/household records were reviewed.
Information gathered during the inspection determined a non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program. The violation was reviewed with the provider during the exit interview.

The record for child 4 did not include the addresses of the two designated persons to be contacted in an emergency if a parent cannot be reached.
The parent will be required to provide the street addresses for the two designated emergency contact persons.
