During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on and completed on 03/06/2026, as a part of the licensure period. The on-site inspection began at 8:45 AM and ended at 9:55 AM. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 4. The inspector reviewed 4 children?s records and 2 caregiver/household member records on-site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition. Unsafe conditions shall include, but not be limited to; splintered, cracked, or otherwise deteriorating wood and protruding nails, bolts, or other components that could entangle or could snag skin.
There were 4 fence boards that have pulled away from the supports revealing protruding nails. Another fence slat was broken off at the bottom.
There was a splintering section of wood at the base of the fence to the left of the yard.
The wooden cover over the utility bin has dry rotted.
The top support bar for the swings is rotting.
Plan of Correction: Provider response:
The fence slats will be replaced and/or nailed back into place.
The piece of wood will be removed.
A new cover will be placed over the utility bin.
A new support beam will be installed from which to hang the swings.
Standard 8VAC20-800-250-A
Hanging items including window blind or curtain cords, appliance cords, and ropes shall be out of reach of children under five years of age.
There was a loose black cable cord and another wire hanging along the garage wall within reach of the children.

All cords will be cur or secured to be inaccessible to the children.
