Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review.

A monitoring inspection was initiated on July 21 2021 and concluded on July 26 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 3 children present, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff/household records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the provider did not obtain, for each caregiver and adult household member, a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Evidence: Record A and Record B had documentation that listed the date of the last screening as June 10, 2019
Plan of Correction: Provider told the licensing inspector that she will go to the minute clinic and get it completed.
