Inspector Notes

An unannounced monitoring inspection was conducted on 09/06/2023 from 03:35 p.m. to 4:00. There were 37 children present, ranging in ages from five to eleven years old with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed virtually on 09/08/2023.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interview, the center failed to keep at the center a separate record for each child.
Evidence:
1. At the time of the inspection, Staff #5 stated that Staff #6 was the only one on site that had access to children?s records and since Staff #6 was not present, the records were not available for review.
Plan of Correction: Student files will be printed and kept on site in the locked file cabinet in the office.
