Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 3/25/2021 and concluded on 3/31/2021. The Provider was contacted by telephone to initiate the inspection. There were 3 children present with 2 staff. The inspector emailed the Provider a list of items required to complete the inspection. The inspector reviewed 2 children?s records, 2 caregiver records and 3 household member records submitted by the Provider to ensure documentation was complete.

The information gathered during the inspection determined non-compliance with applicable standards or laws and violations were documented on the violation notice issued to the facility.

The licensing inspector has reviewed with the Provider COVID-19 Essential Guidance for Child Care programs.

Based on record review and interview, the provider has not obtained documentation of a physical exam for each child within the first 30 days of attendance.
Evidence:
1. Child #1 (date of enrollment: 2/1/2021) record does not contain a physical exam.
2. Child #2 (date of enrollment:10/12/2020) record does not contain a physical exam.
Plan of Correction: I will talk to the parents and request that they bring this document as soon as possible.
