Inspector Notes

An unannounced monitoring inspection was conducted today between 10:20am and 11:15am. There were 4 children present with the Provider and 1 assistant for a total of 12 points within the supervision guidelines. The children were observed playing freely with blocks and cars. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-800-100-A
Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1 (date of enrollment: 9/12/2022) record did not contain a physical exam.
Plan of Correction: I know that the family has it. I'll ask them to bring it today.
Standard 8VAC20-800-480-D
Based on observation, equipment used by children was not used in accordance with the manufacturer's instructions.
Evidence: Child #2 was observed being placed in a high chair and given toys to play with. The provider did not use the shoulder straps to buckle the child into the chair.
Plan of Correction: I don't usually put [them] in that chair. I'll make sure to use the straps when they are in the chair.
Standard 8VAC20-800-70-A
Based on record review, parents were not provided in writing all required information before each child's first day of attendance.
Evidence: Child #1's parents were not provided with the Providers emergency preparedness plan prior to the first date of attendance.
Plan of Correction: I must have forgot to give it to this family. I will give it to them to have.
