Inspector Notes

An on site renewal inspection was conducted on 3/22/2022. There were 81 children present, ranging in ages from 2 years to 12 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-40-N
Based on observation, the facility failed to follow required playground safety procedures including active supervision by staff to include positioning of staff in strategic locations, scanning play activities, and circulating among children. Evidence: on 3/22/2022, Staff A and Staff B sat on a bench together while the preschool children played on the playground and black top area. The staff were not spread out in strategic locations and did not circulate among the children. Staff called out directions to the children while staying seated on the bench.
Plan of Correction: Staff will be reminded to actively supervise children while outdoors.
Standard 8VAC20-780-500-A
Based on observation, the children's hands were not washed with soap and running water or disposable wipes after eating snack. Evidence: on 3/22/2022, the preschool children ate snack on the playground. The children did not wash their hands after eating snack.
Plan of Correction: Children's hands will be washed before and after eating meals or snacks.
Standard 8VAC20-780-560-F
Based on observation, the facility failed to follow the most recent requirements of the USDA food program when providing meals. Evidence: on 3/22/2022, the preschool children were observed having snack. The children were served goldfish, but were not offered a second component from another food group.
Plan of Correction: Two components from two separate food groups will be provided at snack.
