Inspector Notes

An unannounced, on-site renewal inspection was conducted on September 24, 2024. The on-site inspection began at 1:30 p.m. and ended at 3:30 p.m. The inspector reviewed compliance in the areas listed above. There were 20 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Documentation of a fingerprint based national criminal check shall be obtained for each staff prior to employment. Staff 2, employed for over 2 years, did not have a national fingerprint result on file. Staff 2 had a criminal history record from the VA State Police, but did not have a national fingerprint check.
Plan of Correction: Staff 2 will schedule an appointment with Fieldprint for fingerprints as soon as possible.
